Abstract: Employee #1 and two other employees were hauling a 2500-pound automotive alignment rack on a "drywall cart" up an inclined surface. The employees were transporting the rack to the vehicle automotive maintenance facility. The inclined surface of the driveway was measured using a digital level that indicated between a 20 and 22 degree slope. There was an almost flat sidewalk parallel to the driveway. The drywall cart measured approximately 3 feet wide by 2 feet long and had a metal inclined platform tilted toward the railing on one side on the cart. The platform had a spring action so that when the 4 by 8 sheets of drywall are placed on the cart, they lean against the railing. The alignment rack, according to the supervisor, was placed horizontally on the platform and was not secured. Employee #1 and the two other employees were pushing the cart up the inclined surface while the supervisor was holding onto the rail of the cart trying to pull it. The weight of the rack was too much for the three employees to handle and the cart started rolling down the incline. Speculation is that when the cart hit the sidewalk at an angle, it jolted the cart causing the platform to vibrate up and down (in a spring action effect) and causing the rack to flip over. The exact position of Employee #1 is not known, however, the rack fell on the employee's head, killing him.
