Abstract: At about 8:30 a.m. on July 10, 2001, Employee #1, an inspector with Superior Industries International in Van Nuys, CA 91406, was reaching into a moving conveyor to remove a paper towel caught in the area of the conveyor drive belt and pulley. His right index finger was partially amputated when it became entangled in the in-running nip point of the belt and pulley. The Division has determined that this accident was caused by an alleged general violation of California General Industry Safety Order 3314(a) which requires that such machinery be stopped and secured before performing such tasks or that an extension tool be utilized if the machinery needs to remain in operation.
