Abstract: Several employees were preparing a site at the intersection of Thousand Oaks Blvd. and Lake Lindero Drive, Augora Hills, CA for excavation to install a telephone conduit. Employee #1 was holding one end of a string line, used as a paint marking guide, across a portion of the intersection outside the area marked with orange safety cones. Another employee marked the area with paint for excavation. A motor vehicle was traveling northbound on Lake Lindero Drive and ran the red light at the intersection, striking an oncoming truck passing through the intersection. The struck truck spun out of control and struck Employee #1. He was thrown across the sidewalk into the bushes and was killed due to severe head trauma.
