Abstract: On March 10, 2004, an employee was operating a Hyster fork lift. He was removing a load of mixed sized steel beams from one flat bed trailer and placing them onto a second flat bed trailer. This trailer was hooked up to a semi truck. When work began, the fork lift operator and the truck driver were working from the driver's side of the trailer. The fork lift operator was setting a load of beams down but the 40-foot long beams did not match the dunnage. The truck driver then moved to the passenger side of the trailer to reposition the dunnage. When the fork lift operator lifted the load back up, one of the beams flipped off of the trailer. The beam struck the truck driver. He was hospitalized with a broken right leg and an amputation to half of his right foot.
