Abstract: At approximately 9:00 a.m. on January 18, 2002, an employee of Hanson Aggregates went into a tunnel where there is a 24-in.-wide belt conveyor to clean the tail pulley. This belt conveyor delivers rock from storage piles to a hopper for loading rock for concrete. After the employee cleaned around the tail pulley, he saw that the belt was moving over because of build up on the head pulley. He used a 6-in. by 6-in. piece of rubber belting to clean the head pulley. As he did this, his hand or jacket got caught between the belt and the pulley, taking his arm in. Because this head pulley was driven by another belt tail pulley, the other belt pulley began to slip and stopped this pulley. The employee was in this position for 30 to 45 minutes He crushed his right elbow, broke his right thumb, and suffered nerve damage. He was hospitalized for four days.
