Abstract: At approximately 7:15 a.m. on July 19, 2001, a truck driver arrived on the job site and decided to operate the tractor (Case International 4230, Serial Number 960430) to hook up a trailer to transport some pallets. As he was backing the tractor to hook up the trailer, Employee #1 and a coworker came to help. Employee #1 and the coworker were lifting the tongue of the trailer but the tractor operator could not stop the tractor. The rear right wheel of the tractor struck the trailer. This caused the tongue of the trailer to whip around and hit Employee #1 in the chest. The operator of the tractor was not trained in the operation of the tractor. Employee #1 suffered broken ribs and was hospitalized.
