Abstract: At approximately 3:30 p.m. on February 19, 2000, Employee #1, of Back Bay Construction, was instructed by his supervisor to go onto the roof of a building to make some minor repairs. The eaves of the roof were 10 1/2 ft tall. Employee #1 took a 24 ft metal extension ladder and, without extending it fully, set the feet on the wooden floor and ascended the ladder. Having completed the repair, he mounted the ladder to descend when the feet slipped out and he and the ladder fell to the floor. Employee #1 suffered a fractured left leg and right elbow and was hospitalized for 10 days. The wooden floor was wet from a rainfall the previous day, and although the ladder's feet were up to standard, they were not secured.
