Abstract: Employee #1 fractured some bones when he was crushed between a panel, a ladder, and a wall. He was holding a pre-fabricated and pre-casted concrete panel in place while a welder tacked the panel steel to a wall plate. The dimensions of the panel were 30 inches by 9 feet by 5 inches. The panel weighed approximately 2,000 pounds. The panel had been lifted in to position by a fork lift and a blade with a strongback attached to a hoist ring swivel. The swivel shackle bolt required a torque of 100 psi. However, the bolt was not torqued properly. The panel shifted. Then, the bolt became bent. The shackle ultimately came apart allowing the panel to fall, crushing Employee #1 between the panel, the ladder, and the wall. As a result, Employee #1 fractured some bones. He went to a local hospital for treatments.
