Abstract: At 9:30 a.m. on July 12, 1999. Employee #1 fell from a ladder. He was taken to the hospital where he stayed four days. His injures included a broken right hip and broken right wrist. They were painting in a multi-purpose room of an elementary school. The floor in this room was hardwood. Employee #1 was using a 24-foot metal extension ladder. The ladder was extended about 13-feet high on the wall. Employee #1 was taping drywall. One of the rubber pads was missing on the ladders feet. He was up on the ladder about 6-to-10 minutes when the ladder slipped out from under him on the hardwood floor. The ladder was erected about 6 feet away from the wall it was against. Nobody was holding the ladder nor was anything else done to prevent the ladder from slipping.
