Abstract: At approximately 2:55 p.m. on June 22, 1998, Employee #1 was using a table saw to cut a 2-in. strip from a piece of laminate. Laminate is a smooth material that is approximately 0.0625 in. thick. The piece of laminate being cut was approximately 6 in. to 8 in. wide and 5 ft long. The table saw had a guard over the blade but Employee #1 had raised the guard to make this cut. As Employee #1 was feeding the material through the saw, he used a push stick in his right hand and was holding the material with his left hand. Employee #1 did not realize that his thumb was close to the blade and it amputated his left thumb to the first joint. Employee #1 was taken to the hospital, treated and released.
