Abstract: At approximately 11:00 a.m. on February 4, 1998, an employee of James C. Cushman, Inc., fell 22 ft onto a concrete floor trough a 2-ft by 2ft- skylight on the roof on an old barn. He suffered a concussion and was hospitalized. The skylight was newly installed and was made of fiberglass rib sheet.
