Abstract: Employee #1 rode a man-lift to the edge of the wing of a DC-10 to paint a stripe on the fuselage. Employee #1 wore a body harness that was connected to a 5-ft-to-6-ft lanyard. The lanyard was attached from the back of harness to a tie point on the man-lift. Employee #1 attempted to spray the stripe but could not get close enough. Employee #1 then disconnected the lanyard from the man-lift tie point and wrapped the lanyard around the front of the man-lift vertical pipe and leaned out between the upper and mid rail of the man-lift. Employee #1 put his left hand down on the wing for support. The section of wing Employee #1 used for support was covered by brown paper and taped down obscuring a wing flap, which was rotated down in the "open" position. The "open-unsupported paper" could not support the weight placed on it, causing Employee #1 to fall forward out of the basket a distance of 18 ft to the ground. Employee #1 was hospitalized with fractures.
