Abstract: At approximately 9:20 a.m. on October 20, 1997, Employee #1 was helping to fill a formed wall with concrete by bumping it into the form. He was working from a 6 ft stepladder that had been set up on a 17 ft high by 5 1/2 ft wide by 10 ft long scaffold. Employee #1 was on the second step of the ladder when the concrete hose, which was under pressure, flipped up and struck him in the face. He fell off the ladder and onto the scaffold platform. Employee #1 was taken to the hospital, where he was treated and released after five hours. This was the first time Employee #1 had ever worked with a concrete hose. He had not been trained on how to safely handle it or told that the hose pump should have been shut off before he tried to handle it.
