Abstract: Employee #1 was putting tires into a tire cutting machine that was set up to be operated by one person. He was adjusting a tire that was already in the machine when the operator activated the start lever. The cutting blades came forward, amputating Employee #1's his right hand. He was airlifted to UCLA medical facilities for treatment. There were no guards anywhere on the machine. The employer was cited for (1) failure to notify CAL/OSHA of the accident; (2) failure to provide an IIPP; and (3) lack of guarding on the cutting machine.
