Abstract: On September 12, 1996, Employee #1 was working with a wet end press when he noticed some wet cement lodged on the screen. He put his hands into the press to remove the cement, but instead of placing his hands on the bottom, he put one hand through the top of the press. Employee #1's hand was crushed, and he was taken to St. John's Hospital, where he was admitted for treatment. The employer was cited for failure to make an Illness and Injury Prevention Program available at the outset of the investigation, and for failure to protect workers from exposure to the wet press operation.
