Abstract: At approximately 2:00 p.m. on May 17, 2007, Employee #1, a forklift operator, was working in a warehouse near Storage Section B. The floor was marked off into a large rectangle. This area was for storage only and forklifts were not allowed inside this zone. Travel lanes for forklifts go around the perimeter of the storage section, and there are yellow steel support poles just inside the storage section. The employer has an injury and illness prevention program with a code of safe practices, but has not posted forklift rules and employees do not have certification for forklift operation. Employee #1 drove across storage section B, and he did not see the steel support pole, since it was in the same line of vision as the bars on the overhead forklift cage. He was traveling at approximately 5 mph and drove the forks into the pole and straddled it. He was not wearing his seatbelt, and he came up out of his seat and hit the top of his head on the cage. He suffered a 3-in. laceration on the top of his head and was hospitalized.
