Abstract: At approximately 10:00 p.m. on August 27, 2006, Employee #1 was stocking shelves in the outdoor garden department of a home improvement retail warehouse using a nine-step rolling ladder. Employee #1 fell approximately 10 feet off the rolling ladder. Employee #1 was found at approximately 10:15 p.m. at the opposite corner of the room from where he'd been working. He was covered in blood and conscious, but incoherent, with a laceration at the back of his head. There was a pool of blood at the foot of the ladder and a blood trail around the perimeter of the room. Employee #1 was transported to the hospital, where he underwent treatment for a head injury. He died from his injuries on September 19, 2006. It is unclear whether or not lack of illumination played a role in his fall.
