Abstract: On March 11, 2005, Employee #1, a supervisor with Anderson Drilling, was drilling and forming the caissons for the Otay River bridge as part of a State Route 125 highway construction project in San Diego County. The crew set a 14.5-ft I-beam (approximate weight 4,000 lbs.) on top of a newly-drilled support casing. Employee #1 used the winch line on the Eco Worth drill rig to connect another I-beam. Employee #1 was standing near the casing, assisting the crew placing a second I-beam. When the second beam struck the first beam, it fell and struck Employee #1's left leg. He was hospitalized for a fractured leg.
