Abstract: On March 15, 2005, Employee #1, mechanic at a car dealership, was utilizing a Western four post alignment rack, Model Number 12000, to perform a front end alignment on a Toyota truck. The rack elevated vehicles approximately 2 feet above ground. Employee #1 placed the truck on the alignment rack and raised the left front wheel off the rack to make adjustments. As he was kneeling at the front left end of the vehicle, below the height of the rack, Employee #1 lowered the left wheel. The truck rolled slightly forward, causing its front wheels to fall off the front end of the rack and struck Employee #1. Employee #1 was knocked to the floor, resulting in fractures to Employee #1's back at the L 4 and L 5 vertebra. Employee #1 was hospitalized and treated for his injuries. The accident investigation revealed that the brakes on the truck were not set nor were the safety choke wheels set prior to commencing work.
