Abstract: On November 17, 2004, Employee #1, a full-time driver, was asked by his immediate supervisor to clean up the work area, where they park the vehicles. Employee #1 was working alone. There was a stack of steel I-beams with materials sitting on top of them. One of the I-beams was uneven. Employee #1 lifted one end of the beam to align it up with the other beams. He lost his grip on the heavy beam, and it fell down on his finger. The steel I-beam amputated his right ring finger at the first joint. Employee #1 was taken to the hospital by two coworkers, where he was hospitalized for over 24 hours.
