Abstract: On August 11, 2003, Employee #1 was on the trusses of Building Number 4 cutting a 4-ft-by-8-ft plywood sheet and had the plywood straddled between the open-end trusses. The trusses were 4-ft from center to center. Employee #1 was standing on the plywood sheet while cutting the plywood down to a 3-ft-by-5-ft size for a drain. Employee #1 made the first cut with a circular saw to make the plywood 5 ft long. The plywood sheet did not have enough over hang between the trusses and as Employee #1 stepped onto the plywood the sheet collapsed. The plywood sheet and Employee #1 fell 12 ft 7 in. to the concrete floor. Employee #1 was not using fall protection and probably fell onto the edge of the plywood sheet on his landing, due to his torso being cut up and exposing his intestines. Employee #1 also sustained a collapsed lung and fractured 6 ribs, due to this fall. Employee #1 was hospitalized.
