Abstract: At approximately 7:30 a.m. on April 24, 2002, Employee #1 and two coworkers were replacing grinding blades and spacers on grinders following the nightshift. The grinding machines were pavement grinders, Model number PC5000B, used to smooth the concrete road surfaces of highways and bridges. The blades were 14 in. in diameter. The blades and spacers were on a blade shaft that was approximately 7.5 ft-long, weighing 1500 lbs when loaded with all of the blades and spacers. The blade shaft was being held in a vertical position by a base. Employee #1 was handing his coworker the grinding blades from a pallet near the vertical blade shaft. The last blade had been replaced when the other coworker yelled to watch out. Employee #1 tried to steady the tilting blade shaft with his hands, while backing away. The top of the shaft struck his right leg and foot. His coworker attached an eyebolt into the end of the shaft and used a hoist to lift the blade shaft off of Employee #1 and then drove him to a local trauma center. The large tendon on Employee #1's right leg was damaged and he had several surgeries over a six-month period to repair the damage to his right foot.
