Abstract: At 8:10 a.m. on March 5, 2002, Employee #1, a non-contract employee, had been injured at a delivery site. The employer provides roofing materials to roofing contractors. A loading crew normally consists of two employees. Both of the employees involved in this incident were experienced and properly trained in delivery and roof loading procedures of the employer's products. Employee #1 fell from the flatbed of a truck while assisting a coworker in placing a 4-wheel cart on a 3.5-foot-high conveyor that was being used to transport roofing materials from the truck to the roof of a building. The cart that fell from the conveyor to the pavement striking Employee #1 was about 2 feet wide and 4 feet long with large rubber tires and weighed about 200 pounds. It did not have any special manufacturer's model or serial number information on it. The cart is normally placed against the stopped conveyor and slid onto the belt by two employees as one of the first items to go on a roof. The cart fell off the conveyor shortly thereafter and struck Employee #1 on the left shoulder. The coworker contacted his office and Emergency Services was called while another worker at the worksite administered first aid. Paramedics arrived in about 10 minutes and transported Employee #1 to a local trauma center where he was treated for head and chest injuries. Employee #1 was unconscious after the fall to the concrete paving and had no memory of his fall from the truck.
