Abstract: At approximately 5:00 p.m. on November 6, 2000, Employee #1, a 54-year old male employee of Joseph Webb Foods, suffered lacerations on his left leg when the pallet jack collided with a parked vehicle. He was operating a ride-on pallet jack inside a dry storage warehouse. He struck a service van that was improperly parked on the ramp after exiting the warehouse. Employee #1's leg was partially crushed between the pallet jack and the van. Employee #1 was treated at the scene by fire department paramedics and transported to Tri-City Medical Center in Oceanside, California. He had lacerations on his left leg but no fractures. He was released from the hospital approximately twenty four hours later. The incident was reported later the same day by the Vista Fire Department.
