Abstract: At approximately 10:40 a.m. on March 2, 1999, two employees were installing roof truss braces while in a forklift-mounted basket. The basket had been positioned below a steel beam and each employee performed their task on opposite sides of the beam. When the employees had completed their task, they would signal the forklift operator by striking the work platform with a hammer to lower the basket and move to the next beam. The operator heard the signal and backed toward the next beam without first lowering the basket. Employee #1 was pinned between the forklift basket and a steel beam. The other employee began yelling at the operator to stop. The operator lowered the basket and medical assistance was requested. Paramedics arrived in about 5 minutes and Employee #1 was transported to a local trauma center via a helicopter at 11:42 a.m. and was pronounced dead approximately an hour later.
