Abstract: Employee #1, of Schuff Steel, was walking through the main access to the center court, carrying on his right shoulder a tube steel piece that he had just cut. Visqueen plastic sheeting had been placed over the entire walkway, covering two approximately 50 in. square holes in the floor. Employee #1 stepped into one of the holes, lost his balance, and fell to the floor. The tube steel came down and landed on his right middle finger, amputating it to the third quadrant. Two other fingers also sustained lacerations. Employee #1 was taken to Mercy Hospital, where attempts to reattached his finger were unsuccessful.
