Abstract: At approximately 1:15 p.m. on April 14, 2005, Employee #1, a carpenter for Kaiser Permanente Manufacturing Department, was cutting a 37-in.-long piece Plexiglas into 12-in. pieces on a table saw. When he was ready to cut the final piece, he pushed the Plexiglas toward the saw blade with his hand. There was rattling noise coming from the table saw and the Plexiglas kicked back and Employee #1 felt an impact on his left thumb. When he looked down at the table, he noticed that tip of his left thumb was on the table. Employee #1 was transported to Los Angeles County USC Medical Center where he was treated for the partial amputation of his left thumb. The table saw was a Rockwell Uni-saw, Machine Number K46035. The saw blade guard did not completely enclose the saw blade above the table and it was not equipped with an anti-kickback device or spreader.
