Abstract: At approximately 12:00 p.m. on January 19, 2005, Employee #1, a temporary employee of Monarch Consulting working at the Noble Distribution Systems jobsite, was working alone at the southeast corner warehouse receiving area adjacent to bay door 5, operating a Crown model number RR3520-35 stand-up forklift to retrieve linear cardboard sheets at a storage rack area. He was driving the stand-up forklift in reverse, when he inadvertently backed into rack system 052331. His upper body was pinned between the first-level rack system and the stand-up forklift. He sustained an abdominal contusion. That same day, Employee #1 was transported first to the U.S. HealthWorks Medical Group facility and then to Long Beach Memorial Hospital, where he was admitted the same day. He was discharged approximately one week later. An investigation revealed that Employee #1 was not trained in the safe operation of stand-up industrial forklifts.
