Abstract: On November 15, 2004, Employee #1 and a supervisor were loading a large rolling tool box from the warehouse onto a flat bed truck parked in a loading dock. The top cover lid of the box consisted of a two piece hinged assembly that was propped up in the open position and held by two small pins on the frame of the box. During the moving operation the lid became dislodged and fell onto the hand of the injured employee whose hand was positioned on top of the box. Employee #1 was immediately taken to the medical clinic for treatment. Employee #1 sustained amputation of the right index finger due to his injuries.
