Abstract: At approximately 1:00 p.m. on October 6, 2004, Employee #1 and Employee #2, working on a construction site located at a Ralph's Supermarket in Pasadena CA, were positioned approximately thirteen ft above the ground on the elevated work platform of a scissor lift. They were dismantling ceiling wires and pulling off wood beams at the interior north wall of the site. About twenty minutes into the work process, Employee #1 was attempting to gain height to cut a ceiling wire with bolt cutters by standing on the mid-rail when he slipped and fell to the concrete floor. Employee #1 was transported to Huntington Memorial Hospital and admitted the same day, at 2:18 p.m. He sustained a laceration of the abdominal wall, punctures in his head and hands, and a fracture of his left wrist. After undergoing two operations, he was discharged on October 9, 2004, at 12:35 p.m. Equipment Information and Measurements: A Central City Enterprises employee confirmed that the scissor lift involved with the accident is the number 2 scissor lift, a model year 2000 JLG 2033E3 Model 2033-E3, serial number 0200056781 SL-300-2. This has a maximum platform height of 20 ft and a maximum travel height of 20 ft. The rated work load is 750 lbs. The hydraulic system can operate at 3000 psi maximum. It is from Western States High-Reach Equipment Inc., 909-343-2005. It has platform dimensions of 26 in. by 10 ft-10 in. Operator controls at the outside and the inside were present and functioning. The top guardrail height is 39 ft 0.75 in., the midrail height is 22 in., and the toeboard height is 6 in. The tools inside scissor lift at the time of the accident were a Ridgid R-18 Clipper, red with black handles; and a pick with a wooden handle, with no identifiers. The height measured from the north wall ground level to the ceiling area of accident was 22 ft 6 in. The causal factor of the accident was the failure of the employer, Central City Enterprises, to ensure that its employees did not stand on the guardrails of the scissor lift elevated work platform during demolition work. Central City Enterprises is in the business of interior demolition, site clearing and concrete sawing. The specific activity taking place was soft demolition of wood drywall and hard demolition of concrete, masonry, and cement.
