Abstract: At approximately 3:40 p.m. on January 20, 2003, Employee #1 was on a ladder unbolting a steel column that had been installed incorrectly. He fell from the ladder and dislocated his right shoulder and lacerated the back of his head. Investigators believe the steel beam column fell on Employee #1 when he removed 4 of the 5 bolts that were used to secure the steel beam in place. Employee #1 was hospitalized.
