Abstract: Nulab Inc., is primarily a vitamin packing company located in Glendale, California. Employee #1, a machine operator on the capsule polishing machine, had been employed by the company about 5 to 6 years. He had been trained by the maintenance supervisor. Employee #1 was operating the vitamin capsule filling machine when he dropped something and bent down to retrieve it. When he reached for the item that was dropped, Employee #1 placed his right hand on an unguarded moving belt that was on the motor. Employee #1 sustained an amputation of the right pinkie finger and a laceration to his right fourth finger.
