Abstract: At 6:45 a.m. on May 22, 2003, Employee #1, a buffing machine operator for an employer that retreads truck tires, was working to remove a piece of wire that had gotten stuck in the buffing wheel. He turned off the machine and the buffing wheel started to coast into the stop position. As the buffer was still spinning, he grabbed the spinning metal wire with his right hand, causing the wire and his right hand to be drawn into and under the weld of the wheel (the opening between the surface of the buffing blades and the housing). Employee #1 was hospitalized in Rancho Dominguez, CA and treated for a severe avulsion to his right forearm muscle and skin. A detailed investigation revealed that Employee #1 did not properly deenergize and lockout the machine, nor did he ensure that the buffing wheel was completely stopped, before attempting to remove the wire.
