Abstract: At approximately 2:45 p.m. on November 11, 2002, Employee #1 and a coworker, general laborers for a transportation services company that unloads railcars using gantry cranes and industrial trucks, were attempting to move empty railcars from one of the facilities tracks to make way for railcars to be unloaded with a gantry crane. The coworker was operating a Caterpillar industrial truck (Serial Number: 6PD00325) while Employee #1 released the brake of the approximate 9-foot high railcar (Make: Gandola; Model: TFM; Serial Number: 68138) to allow it to be moved. The brake release was located at the opposite end of the railcar from where the industrial truck was to push it, which limited the coworker's line of sight to Employee #1. The coworker began pushing the rail with the industrial truck, not realizing Employee #1 was behind the railcar. A portion of the railcar struck Employee #1 in the head and knocked him to the ground, which then caused the left wheel of the rail to run-over his right foot. The wheels of the railcar were not chocked or otherwise blocked to prevent movement and radio communication was available. Employee #1 was hospitalized and treated for his injuries. The root cause of the accident was determined to be a lack of effective controls to safeguard personnel during railcar movement. Chocks, blocks or other positive means should be used to prevent railcar movement.
