Abstract: At approximately 12:10 p.m., on July 9, 2002, Employee #1, a 44-year old male press operator with several years of experience with M-5 Steel, was assigned to fabricate 10,000 hangers (small metal sheet parts having two 45 degrees bents in the middle of the part with an elongated "Z"-like shape). The employee was using a No. 3 Chicago Press Brake, Model No. 1012L, and Serial No. 20432 that was located in the press department. Employee #1 was bending parts, when he accidentally pressed the foot control while his right hand was within the point of operation. The foot control activated the ram of the brake press to come down, partially amputating the first second, third and fourth finger on the employee's right hand. Employee #1 was transported to USC Medical Center, where he was treated for his injuries and released. A further inspection reveled that the press brake did not have an operational safety guard.
