Abstract: At approximately 11:15 p.m. Employee #1 was performing his duties as a dock worker with a motor line company that distributes and transports goods utilizing freight trucking, industrial truck and manual loading and unloading operations. At the time of the accident, Employee #1 was working alone at loading dock Number 110 of the facility where a bobtail truck had backed in to be unloaded. The loading dock consists of a cement slab and a hinged metal dock plate. This plate measures 6 ft wide and 28.5 in. long when fully extended and is hinged where it connects to the dock. When not extended, one foot of the dock plate is horizontal while the remaining portion hangs vertically from the hinge. On either side of the dock plate are rubber bumpers that prevent trucks from contacting the dock plate when not extended leaving an approximately 6.5 in gap between the truck trailer and dock plate. To bridge this gap, the dock plate is extended into the trailer using a metal hook to lift dock plate up enough so that the hanging portion can be extended to overlap the truck trailer. At the time of the accident, Employee #1 did not extend the dock plate into the truck and proceeded to manually unload the truck. The cargo to be unloaded consisted of crates measuring from 4 ft to 14 ft in length and weighing from 20 pounds to 60 pounds each. When carrying one of these crates from the truck, Employee #1's left foot slipped into the gap between the dock plate and truck trailer bending his foot upwards. Employee #1 suffered a fractured third metatarsal, dislocated great toe and contusion to the left foot. Employee #1 was hospitalized.
