Abstract: At about 11:35 a.m. on December 20, 2001, Employee #1, a journeyman electrician, was installing a lighting system on top of a precipitator. While riding a manlift downwards, his left foot became caught on a landing platform. He attempted to release his foot, lost his balance, and fell about 30 ft. Employee #1 was transported to USC Medical Center, where he was hospitalized for a broken jaw, neck lacerations, five fractured ribs, and a fractured left foot. The cause of the accident was failure to stop the lift prior to getting off. The supervisor gave instructions on how to start and stop the lift, and posted instructions that mandated stopping of the lift prior to getting on or off.
