Abstract: Employee #1 was sanitizing a French bread dough mixer located along the west wall of the plant and adjacent to a metal counter with a sink. The dough mixers are cleaned out/sanitized between dough runs. Employees customarily use a two-step mobile ladder to reach into the dough mixer and scrape dough off the rollers. The ladder measures approximately 20 inches high by 20 inches wide by 20 inches long. It has a safety grating on the step and top step and wheels that retract when the ladder is stepped on. At the time of the accident, Employee #1 was standing on the top step of the ladder, sanitizing the mixer. The employee had just finished scrapping the rollers and was about to step down off the ladder when he fell backward toward the wall and hit his head on the sink and then on the floor. He lost consciousness for 10 minutes and went into convulsions, foaming at the mouth. Paramedics transported him to USC Medical Center where he was unconscious for 2 days, treated for blunt head trauma, and was released.
