Abstract: At approximately 1:00 p.m. on March 14, 2002, Employee #1, a mechanic for Ace Machine Shop, Inc., sustained burns to his face from a propane gas explosion while troubleshooting a propane powered forklift (a Hyster propane-powered forklift, model number SND4D1534) that had stalled in the north entryway to A-1 Refrigeration. The driver of the forklift helped Employee #1 by holding the hood of the Hyster propane-powered forklift up during the troubleshooting. Employee #1 was performing repair work when he contacted the energized points and condenser in the distributor cap located in the forklift's engine compartment causing a spark. The spark ignited propane which had leaked out of the forklift's fuel system and accumulated in the engine compartment. The resulting explosion seriously burned the employee's face. Employee#1 was hospitalized for five days for burn treatment. Employee #1 had been working for four months repairing and maintaining equipment and vehicles. The cause of the accident was due to the presence of an ignition source in the engine compartment of the Hyster forklift where flammable vapors had accumulated.
