Abstract: Employee #1's duties included placing large rolls of mattress tape onto an old finisher machine. The mattress tape would pass through a section that applied a liquid hardener to the mattress tape, which would then pass through the heating system to seal the hardener. As the tape left the heating system, it would then fall into a box on the end of the machine. Employee #1 was using his hands to push down the tape in the box when he contacted a moving belt and pulley drive on the old finisher machine. He suffered the amputation of the third distal of his right middle finger tip. He was hospitalized for his injuries. The belt and pulley drive was unguarded.
