Abstract: At approximately 10:45 a.m. on February 1, 2001, Employee #1, two coworkers and a supervisor were unloading nine granite slabs which weighed approximately 450 pounds each from a shipping container. The accident site was located in a twenty foot shipping container at the outside loading area of the facility. All the granite slabs were placed vertical and adjacent to one another. Beneath the load was an upright wooden pallet. The three workers were positioned inside the shipping container and the supervisor was operating a forklift nearby. When the workers separated the fourth granite slab a few inches from the rest of the load, the load tipped in the direction of the three workers. The workers attempted to hold on to the falling load but immediately stepped away and allowed it fall. During the tipping of the load, the two coworkers received scrapes and abrasions on legs and arms from falling load. Employee #1 was pinned between the load and the inside west wall of the trailer. The two coworkers got out of the trailer. Employee #1 was left inside the trailer. The supervisor immediately used the forklift to lift the load off Employee #1 while the coworkers assisted in pulling him out. Employee #1 expired at the scene. The coworkers were treated at the nearby hospital but were not hospitalized
