Abstract: At approximately 12:45 p.m. on December 29, 2000, Employee #1, a counter pressure operator, was working for a custom wheel manufacturer. He was wearing gloves while he was grinding a 10-in. by 0.75-in bolt to make a pointed tool. The bolt slipped and wedged in between the right side grinding wheel and the chassis of the grinder. His left hand was pinched between bolt and grinding wheel. His left pinky was amputated at above second joint, and he was hospitalized.
