Abstract: On November 9, 2000, Employee #1 was repairing a motorized rolling gate. After finishing his repairs, the gate was reactivated from a remote location. Employee #1 wanted to see whether the gate was working. After ensuring that the gate moved back and forth, and then stopped, Employee #1 pushed on a pulley on the motor assembly. As he pushed on the pulley, the gate activated and Employee #1's fourth left finger was caught in the pinch point between the belt and pulley. As a result, Employee #1's left ring finger was amputated below the distal interphalangeal joint. Employee #1 was transported to Saint Joseph's Hospital emergency room where he was treated for his injuries. The employer was involved in maintenance at the Walt Disney Company. At a remote location, Disney controlled the functioning of the gate and no local lockout and tagout had been performed to prevent the gate from functioning.
