Abstract: At approximately 1:45 p.m. on September 8, 2000, Employee #1 continued making a clear coat lacquer. He checked valves on the pumping apparatus before transferring the resin component of the clear coat lacquer from a 500-gallon portable Tank #1 into 1000-gallon process Tank #2. Employee #1 climbed a portable metal ladder with rubber wheels on bottom, which was positioned adjacent to the Tank #2 and opened the top lid to check inside the tank. As he lifted the stainless steel lid of Tank #2, a source of ignition was introduced to the flammable atmosphere and a flash of fire came out through the open space of the tank, burning his face and both forearms. Employee #1 was hospitalized for three days. Employee #1 sustained second and third degree burn injuries on the face and both forearms.
