Abstract: Employee #1 was operating a Giani Stock Cutter, Style Number 586, Machine Number 2579, a chopper machine. On the day of the accident, Employee #1 says while he was operating the chopper machine the conveyor stopped working. He notified his supervisor. While the supervisor was gone to retrieve some tools, Employee #1 kicked the chain and sprocket with his right foot. Unexpectedly, the chain and sprocket moved, causing Employee #1's right great toe to be caught in the nip point of the chain and sprocket. Employee #1 suffered a crushing injury to his right great toe. After the accident occurred, Employee #1 was taken to Stacy Medical Center where the doctor treated his injuries. On April 26, 2000, surgery was performed, to amputate his right great toe due to complications.
