Abstract: Employee #1 had been working with Elite Manufacture Corporation, a metal frame furniture manufacturer for five and half months. He had operated the cold saw and roll-former machines for five months prior to his worked on the horizontal bender. The horizontal bender is used to bend metal tubes used in seat supports. Employee #1 was trained for about two weeks to operate this bender. The mandrel and dies of this tube bender were set up in the same position for two consecutive days allowing the operator to run the same part continuously. On February 25, 1998, Employee #1 began bending tubes on the tube bender. When he bent the first tube it did not bend to his expectation, it was wrinkled and rough. He called his supervisor and the maintenance people on the job site to check the machine. After resetting the mandrel, the supervisor stood for a few minutes watching the operator process parts. At approximately 7:10 a.m., during the completion of the bend cycle, Employee #1 began to unload parts from the tube bender when the second pressure mandrel activated itself into a clamp position. Employee #1's finger were crushed between the tube and pressure mandrel clamp, then it unclamped freeing his hand. He was transferred to Manhattan Medical Center and eventually lost the tip of his right finger. No one was around the machine or had touched any controls when the accident happened.
