Abstract: At approximately 8:00 a.m. on November 24, 1998, Employee's #1, 2, and 3 were working in the mill area. They were either near or in the pathway of a pressure vessel which exploded. All three employees were hospitalized. Employee #1 and Employee #3 received strains and sprains and Employee #2 received fractures. The accident resulted from a weakened pressure vessel wall and over pressurization of the vessel. Evidence shows that improper welding and patching had been performed on the pressure vessel, that the pressure gauge was not operating properly, and the safety relief valve did not operate properly. Interviews showed that this pressure vessel had not been properly permitted and maintained, the pressure-indicating gauge was not functioning and unlawful welding and patching had been performed on the pressure vessel.
