Abstract: At approximately 2:45 p.m. on December 27, 1997, Employee #1 was using an MPI vertical wax injection machine, model 34-34, serial #0523. He was withdrawing a mold from the machine when it tripped unexpectedly, crushing his left thumb and amputating the tip. He was transported to UCLA Harbor Medical Center, where he was admitted for treatment. The employer is involved in the manufacture of golf components and parts for medical applications.
