Abstract: At approximately 11:15 a.m. on March 26, 2003, Employee #1, working for Powerhouse Entertainment, was retrieving a 0.75-inch-thick, 4-foot by 8-foot sheet of plywood from a stack of 17 sheets. The sheets of plywood were in a rental container truck, standing on end against the wall toward the front of the truck, and secured with a 0.5-inch natural fiber rope tied to the wall ribs of the truck. Employee #1 untied and/or loosened the rope to retrieve a sheet of plywood when the stack became unstable and fell toward the employee. He was crushed and killed by the falling stack of plywood.
