Abstract: Two employees were replacing a glass mirror on a wall in a floral display unit. They were trying to reinstall a conduit and conductors when one of them used a screwdriver to pry loose a conductor that was stuck. An electrical fault occurred, severing the conductor. The employees asked a manager where the circuit breakers were so they could deenergize the circuit. The employees found a circuit breaker that had tripped and assumed that it was the one for the circuit on which they were working. They returned to their work, and one of the employees was pulling the severed conductor from the conduit. Unfortunately, the conductor was still energized, and the employee received an electric shock when he contacted the conductor with his fingers. His coworker pulled him away from the area, and both of them fell to the floor. Cardio-pulmonary resuscitation was administered to the injured employee, but he later died at a hospital.
